Kindergarten and other new student registration for the 2003-04 school year in the Los Gatos Union School District began Aug. 11 for all district schools. Only completed registration packets will be accepted for enrollment.
New student registration forms and materials may be picked up at your home school—Blossom Hill Elementary, Daves Avenue Elementary or Lexington School for grades K-5. Students in grades 6-8 should register at the Raymond J. Fisher Middle School office.
Special notice for Louise Van Meter School Registration: Due to campus construction, registration will be held as follows:
Aug. 11-15: Pick up registration packets and drop off completed packets at the school district office, 15766 Poppy Lane, Los Gatos (behind Daves Avenue Elementary).
Starting Aug. 18: Pick up or drop off registration forms in the new school office. For more information or questions during this week, call 408.335.2250.
Immunization requirements for kindergarten students include:
1. TB Mantoux test
2. Hepatitis B (three doses)
3. Measles-MMR (two doses)
4. Polio (three to four doses)
5. DPT (minimum of four doses)
6. Varicella (or documentation of having had chickenpox)
